If you have not completed 2026 RDC Waiver please print and bring to the Lake. The Lake Phoenix waiver is also required and must be completed on-line PRIOR to arriving.

All participants must be added as guest to be listed on the Lake Phoenix Manifest. RDC will only pay for individuals listed on the Manifest submitted to Lake Phoenix 48 hours prior to the scheduled Diver Day.
